Care Plan Eligibility / Get Started

Care Plans are available to seniors who meet the following criteria:

  • Age 60 or older
  • Living at home, in a facility, or receiving care from family or outside providers
  • In need of monitoring, support, or protection related to daily care
  • Experiencing safety concerns, unmet needs, or signs of neglect, abuse, or isolation
  • Able to participate in a care plan, directly or with help from a trusted person

Eligibility is determined through

  • A brief phone screening or intake interview
  • Information shared by the senior, caregiver, or concerned individual
  • A review of current care routines and known risk factors
  • An initial Quality of Life Check, conducted in person when possible, to help us better understand the senior’s environment and needs

To begin the process

Our team of Advocates responds seven days a week—usually the same day, or within 1 to 2 days when volume is high—to begin the eligibility review.

Financial hardship does not disqualify a senior. If cost of membership is a concern, see our Charity Plan below or Flex Membership options above.

There is no charge for family members who are legally responsible for the senior, including biological or adopted children. Membership must be held by an authorized decision-maker, such as a power of attorney or court-appointed guardian.

We understand that multiple family members may want to be involved in a senior’s care. To ensure clear communication and protect the senior’s privacy:

  • Only one authorized family member can serve as the official point of contact
  • Additional family members may receive updates or support if authorized by the primary member and the senior, without conflicting with privacy laws or the senior’s wishes.
  • The senior’s preferences always take priority when determining who participates in care decisions and communications.

Caregivers / Independent & Living-In

Caregivers play a vital role in a senior’s well-being. Membership provides access to resources, support, and advocacy to help caregivers manage daily challenges and protect the seniors they serve.

Membership benefits include:

  • Regular updates and clear communication about the senior’s care and any changes in condition
  • Assistance with navigating care options, coordinating services, and understanding rights
  • Guidance on advocating effectively while managing caregiver responsibilities
  • Access to trained Seniors Advocates who listen, advise, and intervene when needed
  • Support in maintaining the senior’s dignity, safety, and quality of life

To protect the senior’s privacy and trust:

  • Membership is held by a primary caregiver authorized to make or assist with care decisions
  • Information is shared only with authorized caregivers or family members, respecting legal and ethical privacy standards
  • The senior’s wishes and confidentiality are always prioritized in all communications and advocacy efforts

There is no charge for caregivers who are legally responsible or actively involved in the senior’s care, including adult children or family members providing support.

Who qualifies for charity

Seniors age 60+ facing one or more of the following:

  • Pre-existing conditions or chronic illness
  • Low or no income
  • Unsafe or unstable living conditions
  • Social isolation or lack of family support
  • Current or past mistreatment, neglect or abuse

No lengthy application is required, but we do ask for basic information to assess the situation, confirm eligibility, and ensure we can respond appropriately.

Each request is reviewed individually and handled with urgency.

Many have no one left to speak up for them.

This plan steps in when systems fall short. It offers critical care, protection, and advocacy at no cost to the senior. Funded by donations and powered by compassion, the Charity Plan ensures that no senior faces mistreatment, neglect, or fraud alone.
